Multipoint Oncology Teleconference, Japan

This teleconference has been held on a weekly basis (on Thursday evenings) as a good opportunity for the discussion of important issues in clinical oncology. Nowadays, this plenary meeting among the major cancer centers is indispensable for maintaining the high standard of quality of cancer clinics throughout Japan.

Review (December 2004)

December 2: New modality for the diagnosis of pancreatic cancer (moderated by Dr Imura, Ibaraki Prefectural Chuo Hospital, Ibaraki). Early detection and a qualitative diagnostic approach are important for the improvement of the treatment of pancreatic cancer. There have recently been several new devices and approaches in pancreatic cancer treatment. Dr Imura (Ibaraki Prefectural Chuo Hospital) reported the recent advancement in preoperative pathological evaluation of biopsied specimens using molecular pathological techniques. Dr Nakaizumi (Osaka Prefectural Center for Cancer and Cardiovascular Diseases) showed the current technique of endoscopic cytology of pancreatic juice, ultrasound-guided fine needle aspiration, endoscopic biopsy and curettage of the pancreatic duct, and endoscopic ultrasound-guided fine needle aspiration. Dr Meya (NCC) discussed the differential diagnosis of intraductal papillomatous mucinous tumors by MR imaging. Dr Nakajima (Hitachi Hospital) showed the significance of PET images in the diagnosis of pancreatic cancer, especially for its early detection.

December 9: Cancer treatment for the elderly (moderated by Dr Mochizuki, Iwate Prefectural Chuo Hospital, Iwate). For the treatment of elderly patients with cancer, physical status has the greatest impact on the treatment of choice. Dr Nakano (Iwate Prefectural Cancer Center) discussed the problems associated with surgical treatment of the elderly in terms of preoperative factors related to postoperative morbidity. Dr Fujiwara (Iwate Prefectural Cancer Center) focused on elderly patients with bile duct malignancy accompanied by stenotic jaundice. The importance of endoscopic/percutaneous stent followed by chemotherapy/radiotherapy was shown. Dr Suzuki (Iwate Medical University) discussed the characteristic features of elderly patients from the anesthetic viewpoint, in which the difficulty of preoperative assessment of risk and the postoperative management for early recovery were stressed.

December 16: Prophylactic injection therapy after TUR–Bt for superficial bladder cancer (moderated by Dr Tsukui, Aomori Prefectural Chuo Hospital, Aomori). Although superficial bladder cancer can be managed by TUR-Bt, the high recurrence rate of 70% is an important issue. For the prophylaxis of recurrence, the direct injection of anti-tumor agents and BCG has been performed. Dr Tsukui (Aomori Prefectural Chuo Hospital) reported the efficacy of Pirarubicin (THP) injection after TUR-Bt. Dr Furuie (Hirosaki University) reported on the indications and problems of BCG injection. Dr Kobayashi (Yamagata Prefectural Chuo Hospital) also reported on BCG injection for superficial cancer of the upper urinary tract with curative intent. They performed BCG injection for six cases of pyero-ureteral cancer in situ. However, recurrence and progress of the disease were seen in two and one cases, respectively.
