A Case of Gigantic Fibrosarcoma of the Buttocks Involving the Rectum

A 67-year-old woman was referred to our institute for the treatment of gigantic soft tissue tumor of the buttocks. Magnetic resonance images showed a huge mass measuring 18x14x12 cm in the hip region, with heterogeneous high-signal intensity on T2-weighted images (Fig. 1), and on Gd-enhanced T1-weighted images (Fig. 2). The lower end of the sacrum was broken and the rectum was compressed by the tumor. An open biopsy revealed sclerosing epithelioid fibrosarcoma. The tumor was removed radically together with the rectum, the distal part of sacrum and the entire bilateral integuments of the buttocks, including the gluteus maximus and medius muscles, followed by a permanent colostomy. The uterus was preserved and used as a block between the pelvic cavity and the buttocks. Free Latissimus dorsi musculocutaneous flap covered the whole sacral area. The patient recovered uneventfully. Histological examination made the final diagnosis of sclerosing epithelioid fibrosarcoma.
