A Case of Superficial Hypopharyngeal Cancer Treated by EMR

A 66-year-old man underwent a screening endoscopy following esophagectomy 12 years ago for early esophageal cancer. On conventional endoscopy, a slight reddish area was found out in right pyriform sinus. Using a Lugol staining, the margin of the lesion was delineated clearly (Fig. 1; note that a colour version of this figure is available as supplementary data at http://www.jjco.oxfordjournals.org).

Biopsy specimen taken from the lesion revealed squamous cell carcinoma. As any lymph node enlargement or metastatic lesion was not detected on CT scan, we diagnosed this lesion as a superficial hyopharyngeal cancer (T1N0M0, Stage I). We, therefore, underwent endoscopic mucosal resection using a cap (EMR-C) for this lesion (Fig. 2; note that a colour version of this figure is available as supplementary data at http://www.jjco.oxfordjournals.org). The postoperative course was uneventful. The resected specimen revealed squamous cell carcinoma in situ. After the EMR, the patient is doing well without any sign of relapse on endoscopy and CT scan. Superficial pharyngeal cancer has recently been detected in earlier stage; therefore, such a lesion can be treated by EMR by gastrointestinal endoscopists.
