A Case of Synovial Chondromatosis Involving the Iliopsoas Muscle

A 47-year-old female was referred to our institute presenting with an abdominal lump. Magnetic resonance imaging (MRI) revealed a huge well-circumscribed mass in her right retroperitoneal space, which involved the right iliopsoas muscle (Fig. 1). Open biopsy specimen revealed synovial chondromatosis, which usually arises from the joints. Because the main tumor was apart from the right hip joint on MRI, we concluded that it arose from the synovial bursa around the iliopsoas muscle tendon. We performed an enucleation of the tumor preserving the iliopsoas muscle as much as possible. Macroscopically, the surface of the tumor had irregular, flat bumps, polypoid nodules of various shapes. Microscopically, mature cartilaginous nodules partially covered with the synovial lining cells could be found (Fig. 2; please note that a colour version of Fig. 2 is available as supplementary data at http://www.jjco.oxfordjournals.org). The final diagnosis was the synovial chondromatosis of the hip with extracapsular extension.
