A Case of Lateral Pelvic Lymph Node Recurrence of Rectal Carcinoid

A 33-year-old female presenting with right lower abdominal pain was referred to our institute. She had undergone transanal resection of rectal carcinoid tumor at another hospital 4 years ago. CT scan and MRI at our hospital depicted an 8-cm retroperitoneal tumor occupying the right obturator cavity (Fig. 1, white arrow) and enlargement of mesorectal lymph nodes. Biopsy specimen of the retroperitoneal tumor suggested that the tumor was the recurrence of the rectal carcinoid. We performed low anterior resection of the rectum with right pelvic lateral lymphadenectomy. On pathological examination, the right lateral and mesorectal nodes were found to be composed of oval-shaped cells with round nuclei, exhibiting trabecular patterns (Fig. 2). Immunohistochemical staining for chromogranin A and synaptophysin was positive. MIB-1 labeling index was 5%. Therefore, the pathological diagnosis was the recurrence of carcinoid in the pelvic lateral and mesorectal nodes. The postoperative course was uneventful and the patient is doing well without recurrence 10 months after surgery.
