A 54-year-old man suffered from bilateral nasal obstruction for 4 years and nasal lump for 1 year. He was referred to our hospital for further evaluation and treatment of a cartilage tumor of the nasal septum. The preoperative pathological diagnosis was chondrosarcoma of the nasal septum. Computed tomography showed a large low-density mass with amorphous calcification at the nasal septum, eroding the right maxillary sinus, the right ethmoid sinus and the hard palate (Figs 1 and 2). He underwent subtotal bilateral palato-maxillectomy and reconstruction of palatal and maxillary defects using a free fibula osteocutaneous flap. The pathological diagnosis of the resected specimens was chondrosarcoma, Grade 2, of the nasal septum, with negative surgical margins. However, he developed a local recurrence 15 months after the primary surgery and underwent surgical resection of the recurrent tumor.
